Being well-prepared for your appointment will ensure that Dr. Husman Khan, Dr. Khalil Khan, and their staff have all the information needed to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment.
Please remember to arrive a few minutes before your appointment so paperwork and insurance can be processed in a timely fashion. Be sure to have a picture ID and your insurance card on hand.
Accepted Insurance
As a courtesy, our billing office submits most insurance claims. If you have specific questions about what your insurance plan does or does not cover, please contact your insurance company.
● Please be sure to bring your insurance card and any other applicable insurance information to each appointment. ● Also, be prepared to pay your insurance plan co-payment at the time of your appointment as required by your insurance company. ● Remember that each patient or family member requires a separate appointment and co-payment. If you are not prepared to pay your co-payment, you may be asked to reschedule your appointment.
Refills are best handled by calling your pharmacist, who will contact our office directly. Please allow three business days to process all nonurgent prescription requests.
If you have not seen a doctor recently for a problem, we suggest an office visit. We are unable to process refills after 5 p.m.
Provided below are various forms that require completion before your visit. Please download the appropriate form as directed by your physician and/or staff by clicking the form's name.
Please print and bring the completed form(s) with you, and our staff will have it printed and ready for your signature at your scheduled visit. This will help expedite the registration process. Thank you.
